The practice is open Monday to Friday from 8.00 to 12.30 pm and from 13.30 to 17.00 hours. The practice is closed on Thursday afternoon and is observed by GP De Groot, Tel. 070 511 4561.

Make appointment

The Office hour is by appointment between 08.00-10.30 hours and between 14.00-16.00 hours. To make an appointment for a consultation the practice can be reached by telephone between 8.00-11.00 hours, 11.30-12.30 hours and between 13.30 and 17.00 hours. The practice is closed on Thursday afternoon.

Callback office hours
If you want to talk to the doctor by telephone, you can call the assistants (phone: 070-511 3056).between 8.00 to 10.30 pm and from 14.00 to 16.00 hours. The assistants make a note of where your question is about, so that the doctor can prepare themselves. You will then be called back after 16.30 hours that same day.

Repeat prescriptions
Repeat prescriptions can be requested in two ways:
• Through the recipes line key: 070-5113056 (24 hours a day, 7 days a week)
• By mijngeozndheid.net (24 hours a day, 7 days a week)

For recipes on work days before 11.00 are requested, the medicines are ready the following working day after 15.00 hours at your pharmacy.

Online services

You can make your repeat prescriptions (chronic medication) quickly and directly, 24 hours a day, our online service request via Mijngezondheid.net. No need for a repeat prescription to the practice.
Recently it is also possible to make an appointment for the GP or the Assistant online. This means that you can without telephone contact with the Assistant at any time of the day via your computer or mobile phone to schedule an appointment.
If you want to use the online services you need quite a few things. This is because the online services at a general practice must be well secured and therefore not just accessible.

What should there be arranged?

  1. You must have a DigiD code with SMS control. This can be digitally adjusted via www.digid.nl
  2. Your email address must be in your file by the GP.
  3. The identity check must be done. This means that your Passport, ID or driver's license number-should have been entered in your file.
  4. You must be logged on to www.mijngezondheid.net

Travelers vaccination

Are you travelling and wondering if you need any vaccinations? Also you can do this at our practice. Please contact our assistant!

Quality mark

GPs Van Eijk & Vos have the accreditation mark of the Dutch General practitioners Society (NHG). The independent organization NHG Practice accreditation (NPA) does that only if after thorough screening shows that the quality of medical care and of the practices are set to rural standards. 

NHG Practice accreditation is the quality label for GP practices and we are proud it.


Stay away rate

Unfortunately it happens sometimes that a patient doesn't appear on an appointment, without message. This is very annoying, because we had been able to help another patient at that time. Also means it's time loss and turnover for the GP. The stay away rate will be charged to the patient itself and not with the health care provider. The level of the rate amounts to 10 minutes for a regular consultation of 13 euros. For special operation (such as a surgical procedure) is the stay away stay 33 euros.

By references

You are referred to a hospital or a practitioner outside of general practice? First check your health insurance policy. Some treatments are non-refundable, and sometimes, your health care provider no contract with the hospital. The costs come over for your account!

Electronic medical data sharing

Your doctor and pharmacy keep a file about you. This book contains information that is important for your treatment. Your doctor determines what your symptoms are and what treatments you will get. Register your pharmacist what medications you are getting and for what medications you are allergic. This data can be exchanged with the LSP (rural switching point).

Your doctor or pharmacy makes use of the LSP? He or she will ask you for permission to make available your data through this network.

Another healthcare provider may then use this information if needed for your treatment.

The LSP is divided into about forty regions. All private practices, doctors and pharmacies, that use the LSP, are classified in one of these regions and may be possibly in a different region. For hospitals is one exception. They often have patients who come from multiple regions and can therefore exchange information with multiple regions.

 Do you want health care providers to electronically share your medical data? Then you can give permission to your GP or pharmacist. In addition, you can print out, fill out, and submit the form at your health care provider. Also, you can arrange your consent online on www.ikgeeftoestemming.nl. You can always revoke your consent.
